Hydropneumothorax. This CXR shows a large right pneumothorax (the edge of the collapsed lung is indicated by the arrow). In addition, there is a large pleural effusion, making this a hydropneumothorax. Of note, the ‘meniscus’ sign that we normally see at the edge of a pleural effusion is generally absent when there is an underlying pneumothorax – this can be a helpful sign when dealing with a small pneumothorax which might otherwise be potentially overlooked.
